Under the Gift Aid regulations which came into force in April 2000, it is very easy for members who are UK taxpayers to give more to the benefit of BIOS at no extra cost to themselves. A declaration is made effective by completing this form, or by notifying the Membership Secretary or Treasurer orally or electronically (when this document will be completed by BIOS using your membership details and sent to you as confirmation, though you can still cancel within 30 days). The subsequent paperwork for the tax reclaim is undertaken by BIOS.NOTES You may cancel the declaration at any time by notifying BIOS. Please inform BIOS if you change your name or address, or if you no longer pay sufficient tax (currently 28p for each £1 of your subscription). If you pay tax at a higher rate, you may claim further tax relief in your Self Assessment tax return. If you are unsure about Gift Aid, the Membership Secretary or Treasurer may be able to help. An Inland Revenue leaflet IR113 Gift Aid is available from your local tax office.
If you want to print off this page and complete it as your signed Declaration, then please return it to the Membership Secretary or Treasurer at their address given on the BIOS main page. . Alternatively, you could download this file, and complete and return the Declaration as an attachment - in which case please insert your e-mail address in place of your signature. Please note that this form is not in itself an authorisation for any payment. If you do not already pay your subscription by means of a Standing Order or credit card, it would greatly ease the administration of the Gift Aid scheme if you would consider authorising annual payment, and complete and return a Banker’s Order form or credit card authoristion. Many thanks. |
